You may want to refer to a
tutorial on creating HTML forms

to here, to one of the links,
to the examples,
or
to any other tutorial you find that is good.
Please share your discoveries with the class list.
Add the following to your forms Web page.
- a "hidden" value
- textarea for comments
- select dropdown menu
- select scrolling menu
- fieldset with legend
- color
Once everything works,
use a table to set the entire page up nicely.
